Hospice & Palliative Care

Our Intent…..Prevent and relieve suffering and support quality of life for individuals and their families facing a life threatening illness

 Our Goal…To meet physical, psychological, social, practical, emotional, cultural, and spiritual needs of the resident and family.

Palliative care will be offered to those individuals who are living with a persistent or recurring condition that adversely affects their daily functioning or will predictably reduce life expectancy.

Residents and their families will be provided ongoing education regarding condition changes and disease processes.  They will be involved with the development of the plan of care and set goals for treatment and continued support after the loss of a loved one.

The Susquehanna interdisciplinary team will encompass a holistic approach to ensure that dignity is maintained and quality of life is optimized.

Residents with terminal diagnosis may opt to have a Hospice referral for services in the nursing home.
